Inclusion Monitoring Form

Middle Temple aims to have an inclusive environment for all staff, members, students and service users, by identifying and removing barriers in our practices. Completing this monitoring form will help us achieve this.

While it is voluntary to disclose this information, doing so will enable us to better understand the composition of our workforce/student body/membership and examine our practices fully. 

Your answers will be treated in the strictest confidence, all data disclosed will be stored on UK servers and treated in line with the Data Protection Act 2018 and the UK General Data Protection Regulation. Please note that this information is stored with your individual profile on the Inn’s membership database. In accordance with your communications preferences, you may be contacted to participate in initiatives or events aimed at making the Inn more inclusive. You can opt out of these communications at any time. The Equality Act 2010 defines a person as having a disability if they have a physical or mental impairment which has a substantial long-term adverse effect on their ability to carry out normal day-to-day activities. A long-term adverse effect is one that has lasted 12 months or more or is likely to do so.

The following questions are designed to gather trans data i.e. whether your gender identity and/or gender expression differs from your birth sex. A trans person may or may not seek to undergo gender reassignment hormonal treatment/surgery.
